Vaccine Consent Forms

School-Based Health families, please check your elementary school students' folders for flu shot consent forms. For high school students, consent forms were mailed to home addresses.

Vaccine consent forms are also available at both buildings. Please complete the forms and send them back to School-Based Health in backpacks, or via MyChart as an attached form.

If you would like to be present, please mark it on the consent form, and call School-Based Health to schedule the appointment.

If your child is not enrolled in School-Based Health but you are interested, please reach out to School-Based Health with any questions or concerns.
