We must have the following information to enroll the student *Birth Certificate *Previous School Records *Immunization record *Proof of residency, which can be one of the following: a. A current driver's license with your legal name and current address b. Cover page from a mortgage agreement with the property location and buyers (new owners) names indicated c. A current rent receipt with your rental property's location, landlords name, address and telephone number on it d. A voter's registration card *If applicable, court ordered physical placement/custody provisions for said children/child
